 Resgistration

         

 

As of July 1st the registration is $200 and must be paid in full by end of the first week of practice 8/5/16. Payment plans are available now.

• Completed Western NY Pop Warner ID Registration Form

• Completed Monroe County Redskins Pop Warner Registration Form

• Photo Copy of Birth Certificate

• Current 2 x 3 Picture (must be on photo paper)

• Completed Medical Release Form with Doctor's Signature.(Must be signed and dated after Jan 1, 2016)

• Copy of June 2016 Report Card

• Registration and Raffle Ticket Fees

Players/Cheerleaders who have not submitted all paperwork will not be allowed to practice until the outstanding paperwork is submitted.
